The Whole Person will contact consumers to discuss services and needs for a comfortable transition.

The following will be discussed:

  • Personal Care Assistant (PCA) services
  • Moving destination
  • Needed household items

Need to order a birth certificate? Or perhaps you need a State ID. Whatever document you require for your living situation, we will assist you.

Trusted Transparency

We will help you coordinate with the case manager, your nursing home social worker, and even the landlord or facility. The Whole Person will be with you every step of the way.
